Getting started with Cornerstone Counseling Center

1. Fill out the Intake form on our Get Started page

  • Have your insurance card and personal information for the person to be seen ready

  • Fill out one form for each person

  • Provide details on reason for your visit and best times for an appointment

  • Submit your form and wait to hear from us

2. Create Portal Login & Submit forms

  • Once your client profile has been created you will receive an email with instructions to login.​

  • After logging in and updating your password you can fill out and submit the additional forms needed for your visit. Some of these forms may include:

    • Adult/Child Assessment Form​

    • HIPPA Form

    • Financial Agreement

    • Telehealth Consent

3. Once your forms have been submitted, you will receive a phone call or text message to schedule your visit​​​

Ready to begin?

Your First Session

You can expect your first visit to Cornerstone to take approximately one hour. During this time, your therapist can begin getting to know you and see how we can help.

 

If you are not sure about your specific insurance benefits, deductible or co-pay, please contact your insurance provider prior to your first appointment to answer any questions you may have regarding payment of services.

Fees & Insurance

Session Rates

At Cornerstone we understand that not all of our clients may be working through an insurance provider and may need to choose our self-pay option. Below are our rates for our self-pay clients.

Initial Appointment: $150

Routine Appointments: $125

​

Late Cancellations/No Show Fee: $100 for appointments missed or cancelled with less than 24 hours notice.

Insurance Coverage

Cornerstone Counseling Center accepts most major insurance carriers, and we will file your insurance claims for you. Insurance coverage varies by Counselor. Check your Counselor's profile or call our office for specifics on insurance coverage and copays
